Living Will
A living will addresses your wishes as to the use or continuation of life sustaining support should you become permanently unconscious, terminally ill or incapacitated and unable to make known your wishes. The living will only becomes effective once you lose the capacity to make informed health care decisions. It provides guidance to doctors, hospitals, and loved ones of your wishes regarding treatment, life support, and whether you desire to be an organ donor.

Health Care Power of Attorney
A health care power of attorney provides a third party, the attorney in fact, with authority to make medical decisions on your behalf should you become mentally incapable of making informed decisions regarding your health. The health care power of attorney does not become effective unless you lack mental capacity and, akin to the durable power of attorney, you may limit the scope of authority granted thereunder.
